Try to run something stressing, benchmark or game. But that log stops at 56C, so probably not a GPU temperature issue.

Then check CPU max temperature for instance with HWInfo set to logging when stressed - next suspect for freezing.

Is the 9650M on a MxM card ?
Is the heatsink on properly with heatsink goo?
Still thinking the 9650M GT might be an after market GPU

I heavily suspect it has been replaced - over 10 reviews/spec sheets show only 9600M GT or 9700 M GT have been used. Yes, that's MXM v2 type III.

Its probably a system BIOS issue, probably doesn't know what to do with the card it doesn't recognise.
9630 running latest BIOS ?

Not sure what to suggest the 9650M GT was never meant for the 8930
You'll need to find what laptop it came from and see if there is a driver for it

Did you get the laptop with Win7 installed and working
Because the video card doesn't belong in the laptop, windows and us are struggling to fix your issue
But there must be a driver that does work....somewhere

Had a thought, its a non original card, where do you get these from:

Have look through here, start with ASUS and M50 and X57 series got to ASUS support website and grab drivers for each to try
You will need to modify (remove the SUBSYS number) from INF where ever you see your PCI_ID
